PERMIT APPLICATION INFORMATION

READ ME FIRST

If you are reading this, then you have chosen to begin the process of applying for your Child Development Permit. If you follow these directions, you will speed up your process.

Are you CURRENTLY working in a program with children ages 0-5?

YES
You will need to print the PDF called First 5 Permit Application.

NO
You need to print the attachment called CDD Permit Application.

Is this your first Child Development permit?

YES

You will need:

  • PERMIT APPLICATION:  Print out the permit application you chose from above. Follow all the directions carefully, especially those on  pages 3 and 4. (There may be some parts of the form that do not apply to you.)
  • FINGERPRINTING:  All first time applicants will need to have your fingerprints done. Even if you have been fingerprinted before, you need additional fingerprints for this process. Print out the Fingerprint Information and Directions and the Dual Reporting Form.
  • 41-4: Print out the 41-4 instructions and the PDF called 41-4, following the instructions carefully.
  • Official college transcripts are required from all of the colleges you have attended. Order these early in the process because it might take a couple of weeks.

NO, I have applied before.

You will need:

  • PERMIT APPLICATION:  Print out the permit application you chose from above. Follow ALL the directions. Everything you need is there (including some forms that won’t apply to you).
  • 41-4: Print out the 41-4 instructions and the PDF called 41-4, following the instructions carefully.
  • Official college transcripts are required from all of the colleges you have attended. Order these early in the process because it might take a couple of weeks to receive them.

  • Previous Permit: You will need the original copy of your currently held permit.
