EOPS Application

EOPS will be accepting applications beginning MAY 1st, 2013.

Please contact the EOPS office with any questions about how to fill out the EOPS APPLICATION. Return the completed application to the EOPS office.

CARE Application

All CARE applicants must submit an EOPS application and CARE application and a TANF Certification form.

TANF Agency Certification Form

The TANF Certification Form requests information about your income. It must be submitted with an Agency stamp and signature for CARE application acceptance.

Request Childcare Hours from CARE

Use the CARE Childcare Verification Form to submit the hours needed for study time.

CARE Attendance Sheets

Fill in the CARE Attendance Sheet to submit the hours for your child/children.

Student Change of Information Form

The Student Change of Information Form must be submitted anytime your name, address, telephone number or email changes.

EOPS Petition/Appeal Form

This petition form must be completed prior to meeting with an EOPS counselor. Appeals should be submitted to the EOPS office for review.

Board of Governor's Fee Waiver Application (BOGW)

You must be eligible for the BOGW in order to be eligible for the EOPS program. Please contact the Financial Aid Office for the Fee Waiver form.

Free Application For Federal Student Aid (FAFSA)

Apply for FAFSA to be considered for EOPS or CARE grants and Book vouchers, as state funding permits. Please contact the Financial Aid Office for the FAFSA form.
