Emergency Financial Assistance Program

Dreamkeepers Questionnaire

Developed with support from the Wal-Mart Foundation and with additional fundraising by The Foundation for Grossmont & Cuyamaca Colleges, the Dreamkeepers Emergency Financial Assistance program at Cuyamaca College can provide students with non-recurring emergency funding to allow them to remain in school when faced with unexpected financial hardships. As funding is limited, not all requests can be approved and it is important that you are specific and detailed when answering the on-line application questions and when providing the required documentation to verify your emergency expenses.

Please read the following instructions:
Read each Eligibility Requirement, Application Preparation Requirements, Statement of Need Comments, and Eligible and Ineligible Expenses. as you read
each item check the box next to item.  Checking each box will acknowledge that you have read each of the Dreamkeepers checklist Items. Once you have read all items you will be able to move on to the Dreamkeepers Application pages.

Eligibility Requirements

I understand I must be enrolled in a minimum of 6 units at Cuyamaca College.

I understand I must demonstrate satisfactory academic progress (minimum GPA 2.0) for all completed course work.

I understand I must be seeking a certificate, degree or transfer objective.

Application Preparation

I understand that I must complete the Financial Aid Literacy Course and send the completion notice to the FA Office A300.
I understand that I must complete the Dreamkeepers Budget Planning Worksheet and turn it into the FA Office A300.
I understand that I must prepare all documents showing "extreme personal/financial emergency" consisting of nonrecurring expenses.

Document ALL needs being requested, make sure your documentation is complete:

I will need a Utility Past Due Bill with a designation with a to be shut off Notice
I will need an Eviction Notice, a 3-Day Notice, or A Quit Notice with my rental agreement.
I understand Medical or Dental Unexpected Emergency requests for treatment requires an "Estimates form a Doctor's. or Dentist's Office, it must be signed by the Doctor or Dentist.
Unexpected Automobile Expenses require "ESTIMATES" Only
Child Care Unexpected Expenses "Estimates."

For further individual assistance and guidance in applying for Dreamkeepers, please contact the Financial Aid Office (619) 660-4537 to make an appointment. We are located in the Financial Aid & Scholarships Office (Building 300) at the Student Services Center. Walk-in appointments are available but very limited so we encourage you to call to make an appointment.
